Five factors determine how much you pay for health insurance in Spain. Understanding each helps you find the best value option for your specific situation.
1. Age — the biggest factor by far
Age is the primary driver of health insurance cost. The premium roughly doubles between age 30 and age 50, and increases substantially again in the 60–70 band. This is standard actuarial pricing — older policyholders make more and higher-value claims. At age 65+, premiums are typically 3× what a healthy 30-year-old pays.

2. Coverage tier

Annual vs monthly payment
Most Spanish health insurers require upfront payment — annual or semi-annual. Monthly direct debit is less common and not always available for new visa applicants.

Copayment plans — not NLV compliant
